The Provider is a non-network approved provider with Tricare East. Usual visit fee applies and a Superbill with medical coding will be provided for you to submit to Tricare for partial or full reimbursement.

All Other Plans

Visits are fee for service (self-pay) and a Superbill with medical coding will be provided for you to submit to your insurance carrier for potential reimbursement.
