Add on: Concierge Lab Package (self pay)

Service Description

Lab work is available as an add-on service to a scheduled visit. All-inclusive Concierge Lab Packages are available and include: lab order, draw fee, individualized panel of bloodwork and Provider interpretation. Package is available as an add on to a regularly scheduled visit. COMMON LABS AND PANELS: Anemia Panel Breastmilk Culture Chronic Low Milk Supply Panel Diabetes Screening General Wellness Panel Prolactin Thyroid Antibodies Thyroid Panel All bloodwork panels are individualized to your history, risk factors and health conditions. Cost varies depending on labs and I will invoice you for review. Payment is due at time of ordering but you will always know your cost up front. Pricing is subject to change and quote is good for 14 days. You are welcome to use your FSA/HSA card but I am unable to bill insurance directly or provide a Superbill for cash pay labs. For cancellation of Concierge Lab Package BEFORE labs are drawn, a full refund will be provided minus a $25 cancellation fee. There is no refund after labs are drawn.
